WV Competitor Declaration Form

This form must be completed and post marked by Feb. 16 for Levels 6-10 or March 16 for Levels 4 & 5. Gymnasts not included on this form will not be eligible for the State Meet Competition. Gymnasts should be included regardless if they have qualified as of this date or not. If a gymnast has not qualified, please indicate N Y Q (Not Yet Qualified) in the space provided for meet and date qualified. It is then your responsibility to forward qualifying information ASAP. Please keep in mind the determination of the level that you choose for them is yours and the gymnast's parents' responsibility. Please be conscientious in your level selection as to not have a gymnast work at a level beyond their ability. All gymnasts must use the age determination (see page 3) for all meets for the 2004-2005 season if the scores are to be used for the state meet qualification.
